COUDERSPORT — Cole Memorial Medical Group’s Vice President and Executive Director Timothy Brown announced that Cole Memorial Medical Group earned the National Committee for Quality Assurance (NCQA) Patient-Centered Medical Home (PCMH) re-recognition in October for using evidence-based, patient-centered processes that focus on highly coordinated care.
“This accomplishment speaks to the exemplary work and dedication of our providers, staff and leadership,” stated Brown.
“NCQA Patient-Centered Medical Home Recognition raises the bar in defining high-quality care by emphasizing access, health information technology and coordinated care that is focused on patients,” said NCQA President, Margaret E. O’Kane. “By earning re-recognition, these practices show they have the tools, systems and resources to provide their patients with the right care, at the right time.”
The patient-centered medical home (PCMH) is a model of care that aims to transform the delivery of comprehensive primary care to children, adolescents and adults.
“We embarked on the journey to become recognized for the patient-centered care that we provide within the Cole Memorial Medical Group, back in 2014,” said Kristin Kratzer, director of Population Health and Care Coordination of the Cole Memorial Medical Group. “We recognized the need to adopt a model that would be sustainable and in alignment with our mission to optimize the health of individuals and the communities we serve.”
“Foundational pieces of earning recognition include having a clinician-led care team that provides accessible, comprehensive and coordinated care while focusing on quality and safety. We continue to build on those foundational pieces,” added Kratzer.
