Foothills Medical Group received Patient Centered Medical Home recognition from the National Committee for Quality Assurance for its patient centered, quality and coordinated care.

Earning PCMH recognition means those practices use a model of primary care that combines teamwork and information technology to improve care and patients’ experience and reduce costs. It also fosters partnerships between patients and their healthcare providers. Patients’ care includes coordinated treatment across the healthcare system.

FMG practices earning recognition were the Delevan Health Center and Salamanca Health Center in New York and Bradford Family Practice, Pediatric Associates of Bradford, Smethport Health Center, and Mount Jewett Health Center in Pennsylvania.

“Under the leadership of Jill Owens, MD, FMG medical director, Randi Jorgensen, director, practice management, FMG, Olean, and Sue Pascarella, director, practice management, FMG, Bradford, Patient Centered Medical Home status was achieved in just 18 months,” Finan said.

“NCQA Patient Centered Medical Home recognition raises the bar in defining high quality care by emphasizing access, health information technology and coordinated care focused on patients,” said NCQA President Margaret E. O’Kane. “Recognition shows that these practices have the tools, systems and resources to provide its patients with the right care, at the right time.”
